Restylane®, Radiesse™, and Collagen Treatments at Our Portland, Oregon-Area Office

Finer facial wrinkles and creases begin to appear as a result of sun damage (photoaging) when the underlying collagen and elastin fibers begin to break down and wear out. With time, and yes, age, facial volume is also lost. Smiling, squinting, and facial expressiveness bring about what are called dynamic lines, folds, creases, and wrinkles. This is part of the natural aging process. At the Clackamas offices of dermasurgeon Dr. Peter Goodkin, Restylane®, Radiesse®, and collagen are used for patients from the Portland area and throughout Oregon to overcome these problems.

Restylane®, Perlane®, and Juvederm®

Surface wrinkles and textural changes are best treated with sun protection, ical treatments, and various chemical, mechanical, and laser rejuvenation and resurfacing procedures. Dynamic lines and folds are often best treated with BOTOX® Cosmetic, which temporarily weakens the muscles that cause the problem.

In the past, injectable bovine collagen was the most commonly used material for filling in frown lines, wrinkles, and creases. However, some people were found to be allergic to bovine collagen, and better materials were developed. The use of Cosmoderm™ and Cosmoplast™ (human collagen) has for the most part eliminated the allergy risk. Even so, the effect of collagen does not last long enough for many people.

A natural material that lasts about twice as long as collagen is now available. This material is non-animal-derived hyaluronic acid, a naturally occurring lubricant in our joints. Hyaluronic acid is used in Restylane®, Perlane®, and Juvederm®, which are available at our Oregon practice.

How do Restylane®, Perlane®, and Juvederm® work?

Restylane®, Perlane®, and Juvederm contain hyaluronic acid in the form of a crystal-clear gel. They are injected into the skin to fill the lips, wrinkles, or folds. Once the gel is in your skin it creates volume, resulting in a vital and healthy appearance. When injected into the lips, Restylane®, Perlane®, and Juvederm® can restore a more sensuous and/or natural look.

What areas respond best to treatment?

Fine lines on the upper and lower lip (lipstick lines) smooth and fill nicely with Restylane® Fine Line or Juvederm®. Deeper folds and wrinkles in the cheek areas (melolabial, nasolabial, and drool lines at the corners of the mouth) respond well using Restylane®, Perlane®, or one of the Juvederm® fillers. Lip volume enhancement created by Restylane®, Perlane®, or Juvederm® lasts up to twice the time of standard collagen implant treatments.

What about safety?

Since Restylane®, Perlane®, and Juvederm® are not derived from animals, they don’t pose a significant risk of allergic reaction, and they're safe to use without a skin test. Hyaluronic acid is present in everyone's body. The synthetic, biodegradable hyaluronic acid in Restylane®, Perlane®, and Juvederm® has been used for many years. They vary either in the size of the gel particles or the degree of crosslinking. Thus the products are tailored to the depth of the facial wrinkles or the desired degree of lip enhancement. The use of Restylane®, Perlane®, and Juvederm® for our Oregon patients has been very safe and satisfying. What are the sessions like?

A Restylane®, Perlane®, or Juvederm® treatment session takes less than 30 minutes, in part because no allergy pre-test is needed. For patients who desire it, an anesthetic cream is applied about an hour before the injections. Lip enhancement is preceded by an anesthetic block for upper and lower lips, so that the treatment is nearly painless.

How long do the effects last?

Depending on the area treated, most folds and wrinkles require two treatments, two to four weeks apart. Some can be improved with a single session. The maintenance treatment is not usually required for approximately six to seven months, less for lips. In our experience, these products last almost twice as long as regular collagen injections. Our Portland-area patients have been very pleased with these products.

The wonderful thing about Restylane®, Perlane®, and Juvederm® is that you can replenish your natural beauty whenever you feel the need. Unlike permanent implants, Restylane®, Perlane®, and Juvederm® always give you a choice when the effect of the implant has worn off. More can be added any time you like.

What will I look like right after the procedure?

There's very little discomfort after a Restylane®, Perlane®, or Juvederm® session. You can return to your normal activities right away. The treatment site will have a little redness and swelling, which usually lasts two to three days. Sometimes you can feel a bump in the skin for a few days. For some patients, slight bruising may occur, but it's not extensive and can be hidden with makeup. Depending on the area treated, correction usually lasts from three (lips) to eight months.

Radiesse®

Radiesse™ is made of tiny microspheres of calcium hydroxyl apatite (30%) mixed with a common hydrogel (70%) that is used in many cosmetic products. Calcium hydroxyl apatite is similar to bone and teeth, and it's naturally compatible with your body. However, Radiesse™ is man-made.

Because Radiesse™ is made of biocompatible calcium hydroxyl apatite and a safe, tested hydrogel, no skin test for reactions is needed. There have been no reported reactions from the use of Radiesse™.

Does Radiesse™ become hard or noticeable to touch?

Radiesse™ stays soft and takes on the characteristics of the tissue surrounding the site where it's injected. Soft new tissue will eventually replace the Radiesse™, with most replacement occurring within three months. Patients have reported that Radiesse™ stayed very natural-looking and -feeling.

Do Radiesse™ treatments hurt?

To reduce or eliminate the pain associated with this type of treatment, Dr. Goodkin uses a mild local anesthetic. The numbing effect of the anesthetic wears off over a period of 15 minutes to one or two hours, depending on the type of anesthetic used. There is no pain reported afterwards.

Are there any lingering effects of Radiesse™ treatment?

There is minor inflammation or swelling from the treatment as a result of the expansion of facial tissue. This is normally resolved well within 24 hours but has been reported to last as long as 48 hours for some patients. Bruising is not uncommon and may last a few days. Application of ice intermittently for a short period after the treatment will help reduce any swelling.

When can I return to my normal activities after a Radiesse™ treatment?

Most patients resume normal activities only a few hours after receiving treatment.

Are there any activity restrictions after I undergo a Radiesse™ treatment?

For 48 hours following your treatment, you should avoid activities that strain your facial features as much as possible. For example, avoid activities such as shouting, eating tough foods, using a straw, chewing gum, etc. Otherwise, your normal activities are no problem.

Is Radiesse™ FDA-Approved?

Radiesse™ is an FDA-approved substance but it hasn't been FDA-approved for facial implant use. Radiesse™ is FDA-approved for soft-tissue marking in any anatomical location, for vocal cord injection, and for oral/maxillofacial augmentation. Facial implant use of Radiesse™ is thus considered an "off-label" use of Radiesse™. An FDA panel recently recommended Radiesse™ for facial implant approval. The effects on the body for this off-label use of Radiesse™, both short- and long-term, haven't been determined yet.

Cosmoderm™ and Cosmoplast™ Collagen

So, you looked in the mirror and noticed your first facial wrinkle! Welcome to life and reality. People in their 30s and 40s don't want to show the initial signs of aging. In your 50s and 60s you look in the mirror and don't recognize that person. You feel young and vital but the image doesn't seem to match anymore. The number of cosmetic surgery procedures performed continues to soar, growing at a rate of more than 20 percent each year since 1996. Facial wrinkles and creases begin to appear as the underlying collagen and elastin fibers begin to break down and wear out. While this breakdown is part of the natural aging process, it can also result from normal squinting, frowning, and smiling; smoking can also have significant detrimental effects.

The procedure most frequently performed for filling facial wrinkles and frown lines is collagen treatment. Collagen treatment has been one of the 10 cosmetic procedures performed in the United States since it was approved by the FDA in 1981. Cosmoderm™ and Cosmoplast™ are derived from human collagen tissue. They are used to replace the collagen your skin loses over time. Each is injected below the skin's surface.

Many scars and most wrinkles can be smoothed out with these collagen products. We can also improve the appearance of deeper frown lines and furrows. Collagen injections are a time-tested, noninvasive treatment that provides immediate results.

Collagen injections aren't for everyone

A small percentage of patients (fewer than 3%) can be allergic to bovine collagen. We used to double-skin test before injecting bovine collagen. Most new collagen treatments are done with Cosmoderm™ (human collagen), a substance not requiring skin testing. How are collagen treatments performed?

The collagen is injected beneath the wrinkle, scar, or furrow, and mixes with your own collagen. More than one injection may be necessary. The injections each have a small amount of the anesthetic agent lidocaine, and thus the discomfort experienced with the injection is minimal.

A single treatment or session usually takes less than 20 minutes (it's another "lunchtime" procedure). Puffiness, if present, lasts only one to two hours.

How many treatments will I need?

Dermal defects are typically treated in two or three treatments over a one-to-two-month period, with correction lasting up to one year. Since these treatments don't stop the inevitable aging process, most patients choose to maintain the beneficial effects of these injections by getting touch-ups three or four times a year.

What can I expect after my collagen treatments?

After the treatment, there may be some slight, temporary swelling and bruising that can be covered with makeup. You can return to your normal day immediately. It's rare that anyone has an allergic reaction, but this would most likely be itchiness, redness, and possibly firmness where the injection was made. Depending on the area, treatments usually last from three to six months.

Collagen treatments can be a great option for those who choose not to undergo surgery and for those who'd like to enhance or prolong the results of BOTOX® Cosmetic or laser rejuvenation.
